Agency for Healthcare Research and Quality’s (AHRQ), Center for Evidence and Practice Improvement has announced a national priority to improve the care of patients with Multiple Chronic Conditions (MCC). Patients will MCC’s account for:

  • 64% of all clinic visits
  • 70% of all in-patient hospital stays
  • 83% of prescriptions
  • 71% of national healthcare spending
  • Children and adolescents with MCCs are escalating

AHRQ’s Federal Commitment:

  • Evidence and guidelines addressing cumulative and interactive efforts on MCCs
  • New ways of communicating care overcoming traditional silos and support teamwork
  • Innovative uses of technology bringing evidence to point of care and enable effective care coordination and integration
