Currently, 7 out of 10 Medicare Beneficiaries are living with more than two chronic conditions, or multiple chronic conditions (MCC) and accounts for 86% of Medicare spending. Centers for Medicare and Medicaid Services (CMS), encourages primary care providers to utilize the Chronic Care Management reimbursement codes and participate in the incentive to ensure coordinated care for patients at-risk for disease exacerbations, hospital admission and death.

CMS has just released new resources to support providers, patients and health care practices to fully implement and provide cost effective, comprehensive and collaborative health care to meet the complex medical needs for the largest and fastest growing patient population – those with MCCs.
